Sharing a patient story with us is easy to do and helps bring hope to others. Just follow the simple instructions below:

• Record your story using a camera, webcam or digital camera and transfer it to your computer.



• Then visit YouTube and sign in. If you do not have a YouTube account, you can sign up for a free account.



• Simply upload your video to YouTube, and in the Tag area (as shown below) add the tag "TAFPatients" so that we can identify your video.



• Thats it! We will review your video and notify you within a few days about the status of your video!















By posting or submitting any material (including, without limitation, photos and videos) to us via this website, you are representing: (i) that you are the owner of the material, or are making your posting or submission with the express consent of the owner of the material; and (ii) that you are thirteen years of age or older.
